Nursing Home Medicaid

Dealing with Medicaid when you are also dealing with your loved one's need for long term nursing care, isn't easy. Fortunately, The Rehab Center at Traylor team is here to help.

Alabama Nursing Home Medicaid is the only form of Medicaid that will pay room and board for long term nursing home care. Information on this page is only provided to assist you with this process. Final Medicaid approval can only be done by the Alabama Medicaid Office. But remember, our team is here (334-749-1471) to help you to assess your financial situation and assist you in filing the application.

The following information MUST be provided to assess someone for Alabama Nursing Home Medicaid. 

NOTE: This is not an all inclusive list of verification that may be needed.  Each case is individual and specific verification may be required during the application process.

  1. Copies of Medicare, Medicaid, and Social Security Cards

  2. Verification of the gross amount of Social Security, VA, Railroad, CSF checks, private pension checks, and rental income.  Verification should include claim and/or identification numbers.

  3. Copies of bank statements (Any and ALL ACCOUNTS for prospective resident AND spouse) back 5 years from the month before admission to the nursing home.

  4. Verification of any current CD's, IRA's, and Savings Bonds or cashed in the past 5 years

  5. Verification of current stocks, bonds, mutual funds or cashed in the past 5 years.

  6. Copies of deeds to property currently owned or in which you or your spouse have an interest (heir property, life estate).  Copies of original and transfer deeds to any property previously owned.

  7. Copies of trusts, mortgages, loans, and promissory notes.

  8. Copies of ALL insurance policies:

    1. Life, burial, funeral, vault, casket, cash, term and/or group.

    2. Nursing Home policies

    3. Health, hospital, and/or cancer policies.  A copy of the card or premium notice and copy of payment method is needed.

  9. Copies of pre-need, pre-paid, pre-arranged burial contract including itemized list of charges.

  10. Copy of power of attorney, guardianship papers, or curator papers.
